DCMH pharmacist receives state award

(Greensburg, IN) – Decatur County Memorial Hospital’s Ambulatory Care Pharmacist Matt Weber, PharmD, recently received the Patient & Family Experience Award from the Indiana State Office of Rural Health.

Annually, the Indiana Department of Health’s State Office of Rural Health awards those making a difference in rural healthcare for their outstanding achievements.

As an ambulatory care pharmacist, Weber meets with patients who need consultation regarding their prescriptions and managing their chronic conditions, including diabetes, hypertension, and obesity.

In his role, Weber collaborates with DCMH providers to create individualized care plans for each patient. In 2022, Weber established a program that provides individualized education and information to patients. Weber received this honor for his visionary efforts to provide diabetes education and support for his patients and their families.

Weber accepted the award during the presentation. He thanked the Indiana Department of Health’s State Office of Rural Health for the recognition, adding, “We’ve had great success, and it is really exciting to see the improvements in people’s health and blood sugar and their engagement once they understand what diabetes is and how it impacts them.”
