(Greensburg, IN) – At her routine wellness appointment, Dorene Greiwe’s life took an incredible turn, thanks to one pivotal introduction.
During the visit, Dr. Jami Rayles introduced Dorene to Matt Weber, PharmD, CDCES—an Ambulatory Care Pharmacist at DCMH—who would become her partner in transforming her health. Before their meeting, Dr. Rayles noticed Dorene’s A1c levels had been elevated for some time, putting her at higher risk for complications like nerve damage, stroke, and kidney failure. Dorene had not yet fully incorporated significant dietary or physical activity changes, which played a role in her previous challenges with achieving results. Starting in November 2023, Dorene and Matt worked together one-on-one, setting goals and creating a plan that was both manageable and effective. Matt’s personalized support, from providing recipes to keeping her accountable, made all the difference. By November 2024, Dorene’s A1c had dropped from dangerously high levels to an impressive 6.4, dramatically lowering her risk of complications.
“It wasn’t one big change but a combination of small, sustainable steps,” Dorene shared. “Matt educated me on how insulin works, helped me adjust my diet, and ensured my medications were not only correct but also affordable.” Through her dedication to improving her diet and exercise habits, along with Matt’s guidance, Dorene achieved incredible results—losing over 30 pounds and gaining more energy than she’d felt in years. While she was on medication to help manage her diabetes, Dorene’s weight loss was achieved entirely through lifestyle changes, without the use of medications specifically designed for weight loss. Along the way, she developed a new appreciation for healthier foods and exercise, all while continuing to enjoy the meals she loves.
Matt also monitored Dorene’s progress remotely with a continuous glucose monitor, reaching out proactively to discuss spikes or setbacks. “He was there for me every step of the way—answering questions, addressing frustrations, and helping me keep going,” Dorene said. The encouragement didn’t stop there; her family and friends rallied around her too. Her colleagues brought health-conscious meals to workplace pitch-ins, her daughter joined her for exercise sessions, and meal prepping became a family affair.
Now, with her health and mood greatly improved, Dorene feels ready to embrace the next chapter of her life fully.
