JAC-CEN-DEL Kindergarten Registration
April 4, 2019 @ 1:00 pm - 6:00 pm
Kindergarten registration for Jac-Cen-Del Elementary School will be held on Thursday, April 4th, 2019 from 1:00 PM to 6:00 PM. All children must be five years of age on or before August 1st to attend kindergarten during the 2019-2020 school year.
Parents and registering kindergarten students are requested to attend. Parents will meet with the kindergarten teachers and will also meet with the school nurse to discuss the child’s immunization record and any health concerns. The transportation director will be available to discuss bus routes.
You must bring your child’s official birth certificate. The certificate from the hospital is not acceptable. Parents must also bring the doctor or clinic record of the child’s immunizations.
The Ripley County Health Department will be happy to assist in giving any vaccines needed for your child. You may call 689-0506 to make an appointment.
Margaret Mary Community Hospital will host immunization clinics (by appointment only) on Tuesdays, Wednesdays, Thursdays, and Fridays from 7:30AM – 4:30PM, and Mondays from 7:30AM – 6:00PM. They are located at 1051 SR 229 (across from Krogers). The phone number is (812) 932-5105.
Decatur County Memorial Hospital in Greensburg holds an open shot clinic every Tuesday from 3:00PM – 6:00PM at the Medical Arts Plaza Center. No appointment needed. Ripley County residents are also welcomed at this clinic. The phone number is (812)-222-0422.
There is no charge to Ripley County residents for shots at any of these sites if you do not have insurance, or if you are on Medicaid, or you have insurance that does not cover vaccines. If you do have insurance coverage for vaccines, Margaret Mary Community Hospital, Decatur County Memorial Hospital, and the Ripley County Health Department will be able to bill the insurance company for you. You must bring your child’s shot record with you when you go to the clinics.
You may also contact your primary healthcare provider for vaccines.
Your child will not be officially registered for kindergarten until an official birth certificate and an updated immunization record are on file at the school.
